I hereby give my voluntary consent to the child's participation in the activities of ADSC 2019 diving or swimming class and agree to all conditions of enrollment in the class.
I acknowledge that while Alberta Diving and Swimming Club, its Staffs and volunteers, will make every reasonable effort to minimize exposure to known risks, all hazards and dangers associated with these activities cannot be foreseen or may be beyond the control of the Club, volunteers, and staff. I agree to waive any claims of liability that may arise against any organization or personnel relative to the above.
I authorize club staff and volunteers to provide and obtain any and all medical attention in the event of a medical emergency.
I give Club staff and volunteers the authority to act on my behalf in the case of an emergency, and authorize treatment if necessary -- acknowledging that the emergency contact listed above will be notified as soon as possible. I understand that I am financially responsible for any health care.
I hereby grant full permission for ADSC to record any or all of my child's participation in this event via photos, videos, or any other kind of media for the purpose of future camp promotion without any reimbursement of any kind due to me or my child.
